We are pleased to inform you that we will be offering optional lunches for your child. These are carefully prepared by Chef Anne-Marie Bilokrely with many years of experience in a childcare environment. For your convenience, we will offer a feature meal each day, a vegetarian option and an every day lunch menu.

HOW TO FILL OUT LUNCH ORDER FORM

1. Please fill in your child's name and class (1 form per child).
2. Choose one meal and indicate with an 'X' under the appropriate column for portion.
3. Tally Every Day, 'C', 'E' and 'A' orders and fill out bottom (double-check).
4. Calculate orders and fill in TOTAL PAID.

6. Take a photo or make a photocopy of form prior to submission to avoid conflicts in order.
7. Submit form by email (orders submitted late will be notified of changes).

NOTE: Emergency lunches will be charged $6.00 and will be limited to what is available from the daily menu only. No exceptions.
